Survey
Dear Parent or Guardian,
St. Dennis is always working to improve the ways that families and schools can help each
other—and help all children do well in school. We would like your ideas about this and invite
you to participate in a school survey. We will use your responses to learn about our students
and families and to plan new projects. To do the best job, we need responses from every family.
To help us gather information, Anne Walsh, a 4th year Doctoral student in School Psychology at Loyola
University Chicago, is conducting research at St. Dennis for purposes of her dissertation. Should you
decide to participate in the project, you will be asked to complete an on-line survey that is estimated
to take no more than 10 minutes to complete. The survey will ask you about the ways you are involved in your child's education and your child's
mental health needs. Your participation would be greatly appreciated and your responses will be
confidential and anonymous. More information about the project and about participating in this
research can be found on the first page of the survey, which can be accessed by clicking the link
below.
If you are interested in participating, please click the link below. Once you click this
link, you will be taken to a secure site to complete the survey. Your IP addresses will be suppressed
to insure there is no way you can be identified. Both mothers/female primary caregivers and
fathers/male primary caregivers are invited to participate. If you have any questions, please
contact Anne as awalsh3@luc.edu. Moreover, should you have any questions about your rights as a
research participant, please feel free to contact Loyola University's Compliance Manager
at (773) 508-2689.
Thank you for your voluntary participation and for helping us with research at St. Dennis!
